What to bringFor medication reviews: all prescription bottles (or a list), any OTC meds, supplements, and your insurance card. New patients may also bring a completed patient intake form. For prescriber consults: a brief, de-identified summary of the clinical question — no patient names, initials, dates of birth, or Rx numbers on this form.

Questions

Is this really free?

Yes. We don't bill insurance for consultations. Many people who come in for a review end up filling prescriptions with us — no pressure either way.

Can a family member join the call?

Of course. Mention it when you call to schedule. Three-way phone is no problem; for in-person, bring whoever you'd like.

What if I need to reschedule?

Call (323) 348-4205 anytime. We ask for a heads-up so we can offer the slot to someone else.
